You may file for unemployment insurance in person at any Department of Workforce Services office, by calling the TeleClaim Center, or on forms for filing claims by mail, which have been placed in most public buildings.
Qualifying for Unemployment Insurance:
To be eligible to apply for Unemployment Benefits, you must:
Be free of disqualification
Be unemployed
Be available for suitable work
Be physically and mentally able to perform suitable work
Be free of participation or direct interest in a labor dispute
Make a reasonable effort to find work
Be a US citizen or legally authorized to work in the US
In most States, a waiting period of one (1) week must be served before benefits are payable.

Social Security number
Mother's maiden name (for security/access purposes)
Driver's License number (if you have one)
Veteran/Military separation date
Name, address, phone number of your last employer
Employer's Federal ID Number (from your pay stub or W2 form)
How much you earned
Date started and date ended employment
Previous employer information. Usually, the last 2 years of employer information is required.
