Qualifying for Unemployment Insurance:

To be eligible to apply for Unemployment Benefits, you must:

• Be free of disqualification
• Be unemployed
• Be available for suitable work
• Be physically and mentally able to perform suitable work
• Be free of participation or direct interest in a labor dispute
• Make a reasonable effort to find work
• Be a US citizen or legally authorized to work in the US

In most States, a waiting period of one (1) week must be served before benefits are payable.

Information Needed to File for Unemployment:

• Full name, Mailing address and phone number
• Social Security number
• Mother's maiden name (for security/access purposes)
• Driver's License number (if you have one)
• Veteran/Military separation date
• Name, address, phone number of your last employer
• Employer's Federal ID Number (from your pay stub or W2 form)
• How much you earned
• Date started and date ended employment
• Previous employer information. Usually, the last 2 years of employer information is required.
